Job Summary
The Community Based Patient Care Coordinator (PCC) uses a collaborative approach through which patient care is organized to support the best practice, desired patient outcomes and efficiency during a patient's episode of care. The PCC operates under a philosophy in which a registered nurse is accountable for planning, coordination and maintaining continuity of patient care and for maintaining a relationship with the patient, family and physician throughout the time of admission to a particular unit of care. The focus includes advocacy for the patient, clinical competence and stewardship in the use of resources.

Qualifications

Registered Nurse (RN) Must meet the qualifications of the Community Based Services RN.
Current MN Registered Nurse License.
Driver’s License required.
Reliable transportation to make client home visits.
